Tuesday, February 07, 2012 ( 2:25 AM ) shut up christine i usually go to bed around 8 or 9pm but i had coffee and tea earlier today and now i can't sleep so what do i do with all this energy? jump rope (without a rope), eat 90% of my new block of cheese (i just opened it yesterday! fatty!) and write my Advance Directive seriously though. if you don't have an Advance Directive yet, you should think about it as a courtesy and gift to your loved ones so that they won't have to wonder if they made the right choices i choose Makoto as my agent because i trust he will carry out my wishes and act on my behalf in the event of my incapacitation (huge responsibility. thank you makoto) it's not always a good idea to choose your spouse as your agent, because it can get emotional and such. don't get emotional on me, okay makoto? :) here is an electronic copy for easy accessibility in emergencies Directive made this 7th day of February in the year 2012 I, C.B., being of sound mind, willfully and voluntarily make known my desire that my life shall not be artificially prolonged under the circumstances set forth in this directive. I do not want my life to be prolonged and I do not want life-sustaining treatment to be provided or continued: 1) if I am in an irreversible coma or persistent vegetative state; or 2) if the application of life-sustaining procedures would serve only to artificially postpone the moment of my death; or 3) under any circumstances where the burdens of treatment outweigh the expected benefits. In the absence of my ability to give directions regarding the use of those life-sustaining procedures, it is my intention that this directive be honored by my family and physicians as the final expression of my legal right to refuse medical or surgical treatment and accept the consequences from that refusal. This directive is in effect until it is revoked. I understand the full import of this directive and I am emotionally and mentally competent to make this directive. I understand that I may revoke this directive at any time. putitinwriting.org free state-specific advance directive forms #
